General Policies
Appointments and Arrival Times
Please schedule appointments by calling our office at (615) 883-9595 or going online at www.AndersonEyecare.com.
So that we can get you checked in on time, we ask that you arrive at least 10 minutes prior to your scheduled appointment time. For your convenience, we provide online and downloadable forms for you to complete ahead of your appointment. Completion of online forms prior to you appointment will expedite your check in process.
Please give 24 hour or more notice for all appointment cancellations. This 24 hour or more notice may allow us to fill your appointment time with someone on our wait list. We understand that our patients are busier than ever and unavoidable conflicts may occur. However, as a courtesy please notify us as early as possible.
Our policy regarding cancellations within 24 hours or missing an appointment without notice is as follows:
First appointment missed:
No charge (However, future high demand appointment times may be limited).
Second appointment missed:
No charge (However, scheduling for a future third appointment will require a $25.00 non-refundable deposit.)
If you keep this third appointment:
Your $25.00 deposit can be applied to any charges you incur during your visit.
If you do not keep this third appointment:
Your $25.00 deposit is recorded as a no show/cancellation fee in your financial record. This $25 deposit is required for all future appointments until an appointment is kept.
Compared to most healthcare offices who charge a “last minute cancellation fee” on the first occurrence, our office policy is very forgiving. However, we must consider the needs of our patients on our wait list.
Please note: The $25.00 deposit is only lost if a patient schedules and misses three (3) appointments. This prevented three other patients from being seen.
Late Arrivals
Our office strives hard to see our patients in a timely manner. Therefore, if you arrive 15 or minutes late, we may have to reschedule your appointment.
What You Should Bring to Your Appointment
Please bring an insurance card to your appointment. We also need the social security number of the guarantor so we can file the insurance claim. Failure to provide this information will require payment for the full cost of the appointment and any materials (i.e. frames, lenses and contact lenses) ordered at the time of service.
Please also bring your most current pair of glasses and if you have them, prescription sunglasses. If you are a contact lens wearer, please bring a copy of your contact lens prescription and/or your contact lens boxes with you.
Insurance
We accept most major commercial insurance carriers including Medicare. However, we do not accept Tenncare, Amerigroup or Medicaid.
We accept most major vision insurance plans. However, we do not accept Spectera (United Health care Vision), Davis or Block Vision.
If a patient’s insurance plan requires a referral, it is the patient’s responsibility to confirm that their primary care provider has provided a referral prior to your visit.
Before patients arrive in our office, we attempt to verify insurance coverage. If the benefits cannot be verified, the full charge of the visit must be paid before leaving the office. Although we provide this research as a courtesy, it is still the patient’s responsibility to check with their insurance plan regarding their coverage. Additionally, it is important to confirm that Dr. Anderson, with whom you have an appointment, is in network with your plan.
Minors
Minors (ages 17 and under) must be accompanied by their legal guardian to their visit.
Transfers from Other Doctors
If you are transferring your care from another physician to our practice, please have your medical records faxed to our office at least 48 hours prior to your appointment. Our fax number is 615-883-9691. This will prevent any confusion regarding previous treatments.
Prescriptions
Prescriptions will be filled during business hours and may take up to 2 business days to process. Please leave a message for our technicians and they will return your call by the end of the following business day.
Hours
Our providers are only available during normal business hours. If you have an emergency that has to be addressed after hours please call 615-883-9595. Listen to the message carefully and follow the directions. When redirected, please leave a message that includes your name and a phone number where you can be reached.
